Health insurance is required for all EGSC students in order to enroll for classes. This may be insurance that students bring with them (family, employer, etc.), insurance they purchase through the school, or a combination of both plans. The University System of Georgia has partnered with United Health Care to offer health insurance to all USG students. There are two types of plans – mandatory and voluntary. International students fall under the mandatory plan and must be insured.
As in past years, all students will need to go online and either enroll in the insurance plan or complete a waiver by showing proof of other insurance coverage. The enrollment/waiver system is available here. There are two steps, first students must register and create an account and then complete either the waiver or enrollment process. Students must complete the process each semester they are a student.
We are encouraging students to complete their waiver by July 1 for Fall, December 1 for Spring, or April 1 for Summer so that the charge will never show up on their tuition bill.
All students are required to turn in the Immunization & Medical History Form to Student Affairs. The state of Georgia requires all students to be in compliance with the immunization requirements or their registrations will be dropped. In order to avoid registration issues, please submit completed forms prior to attending Orientation. Here is information regarding required and recommended immunizations per the USG.
Return your completed forms and/or immunization records to Enrollment Management by mail to:
East Georgia State College or fax to
131 College Circle, Swainsboro, GA 30401 (478)289-2353
If your doctor's office does not provide a standard form of your immunization history, they may complete the East Georgia State College Immunization Health History Form located here.
It is strongly recommended that you also BRING a copy of your forms and records with you to New Student Orientation. Please here to select the correct Immunization form for the campus you will be attending.
